HOW TO APPLY

APPLY FOR AN ASSISTANCE DOG

If you are interested in an assistance dog, please call 404.735.3647 to request an application packet or download an application packet online.

The application requires that you provide a physician's recommendation and write a letter explaining why you want a service dog.

After processing your application, GCI will conduct an interview by phone or at our training facility in Woodstock, Ga. Secondary interviews in your home and/or secondary physician referrals may be requested. Selected applicants will be placed on our waiting list.

Most applicants wait many years to receive a canine partner. Dogs are matched with potential recipients based on many factors including needs, personality, energy level, and lifestyle. Finding the right dog for you, one that fits harmoniously into your life, is critical to producing successful service dog teams.

GCI's current program emphasis is to provide trained service dogs for children and adults with physical disabilities.

WHAT ARE THE GENERAL REQUIREMENTS TO RECEIVE A SERVICE DOG?

1. You must be able to attend a two week training camp in Woodstock, Ga. to receive proper instruction on how to care for and work with your dog.

2. You must complete an intensive certification process, which includes a written final, an oral exam, and multiple dog handling tests.

3. You must demonstrate an ability to take care of your dog's physical, mental, and emotional needs.

4. You must participate in frequent training lessons and evaluations given in your home for the first year, which is a probationary period.

5. At the end of the first year, your team will be re-tested to determine permanent placement of the service dog.

6. Each service dog team is re-tested every 18 months to maintain public privileges.


WE REALLY CARE! Our support doesn't end when we place a dog with you. For as long as you are partnered with your dog, our trainers and volunteers will be there to help your team succeed.
